When contacting us for an appointment, please have the following information ready:

• Your full name, date of birth, and contact information
• Your insurance information and ID number
• The reason for your visit or type of specialist you need to see
• Any preferred dates and times for your appointment
• Any referral information from your primary care physician

To reschedule or cancel an appointment, please call our appointments line at (787) 268-4172 at least 24 hours in advance. You can also email appointments@santurcemedical.com with your name, appointment details, and reason for rescheduling or cancellation. Please note that late cancellations (less than 24 hours notice) may incur a fee.

Yes, we have a paved parking area with approximately 26 spaces available for patients and visitors. The parking is located on-site and is free of charge for patients with appointments. Additional street parking is also available nearby if our lot is full.

To request a copy of your medical records, please complete a Medical Records Release Form, which can be obtained from our reception desk or downloaded from our website. Submit the completed form to our Medical Records Department in person, by mail, or by email to records@santurcemedical.com. Please allow 5-7 business days for processing. There may be a nominal fee for copying and processing records.

• Medicare and Medicare Advantage plans
• Medicaid (Mi Salud/Plan Vital)
